FRSP Client Handbook

The purpose of this FRSP client handbook is to provide families with a quick reference guide to program information regarding the Family Re-Housing and Stabilization Program (FRSP). This handbook contains information in regards to the following;

FRSP Client Acknowledgement

Client handbook acknowledgement page only. The FRSP client signs the acknowledgment page and the Provider uploads the signed document in HMIS.

Family Extension Request Form

This form is completed at the time a client requests an extension. When completing this form all criteria that is applicable to the family must be checked and supporting documentation must be provided. This form is signed by both the client and the Case Manager.

Family Budget Worksheet

This form is used monthly to capture the household income. This form is signed by both the client and the Case Manager.

Reasonable Modification Request Form (Reasonable Accommodation)

This form is completed when modifications are necessary to avoid discrimination based on disability, unless the provider demonstrates that the modifications would fundamentally alter the nature of services. (D.C. Official Code 4-754.11(a)(3). This form is signed by the Case Manager, Client and TCP.

Relocation Acknowledgment Form

Upon determination TCP the Case Manager will issue this notice to the family within 24 hours to acknowledge that they have received a decision to their request. This form is signed by the client.

Family Extension Determination Notice

This notice is issued within 10 business days of the client requesting a formal written extension. This notice is signed by TCP and the client.

HAP Contract

Please fill out the HAP contract if you are interested in enrolling in the Rental Partnership Initiative.

Notice of Cessation

Please fill out the HAP contract if you are interested in enrollinThis notice is issued to the client to explain the reason the Provider is exiting the family from the program. This notice entails the reason, date and final subsidy payment amount.g in the Rental Partnership Initiative.

FRSP Service Checklist

The Checklist shall reflect the entire length of service the family has been in the program and the tasks that were completed during that time.

FRSP Voluntary Withdrawal Form

Form for FRSP families who would like to voluntarily exit the FRSP program. Note: Any family who receives more than a nominal amount of rental assistance is not a good candidate for a voluntary withdrawal.

Release of Information

This document allows DHS/FSA to share client information with DHS/FSA partners and government entities listed in the document for the purpose of providing homeless services in accordance with the HSRA.

Family Transfer Summary ( Non-TAH or PSH)

This document should be used for families who are being transferred from a program within the continuum that is NOT Targeted Affordable Housing (TAH) or Permanent Supportive Housing (PSH). For example, this should be used for transfers between emergency shelters, to transitional housing or when requesting expedited case management for FRSP.

This document should be used for families who are being transferred to Targeted Affordable Housing (TAH) or Permanent Supportive Housing (PSH).
